I'm using Novice Form (www.noviceform.com).

I have the line in the HTML form:

When testing it works fine if I fill in all above form fields. If I don't fill in any of the above fields I get:
Missing form fields!
The Below Required Fields Where Left Blank:

and it does not list the fields that were left blank.

Any ideas what I'm doing wrong?
